Adult Foster Care (AFC) Program Requirements

Our state-funded Adult Foster Care program enables seniors and individuals with disabilities to remain in the comfort of their homes, supported by a dedicated caregiver who provides personalized care.

Eligible Participants

To participate in the Adult Foster Care program, individuals must:

  • Have MassHealth or CommonHealth insurance
  • Be at least 16 years of age
  • Have a medical diagnosis or require assistance with one or more Activities of Daily Living (ADLs), including:
    • Bathing
    • Dressing
    • Eating
    • Toileting
    • Transferring (in/out of bed; up/down from a chair)
    • Ambulation
    • Management of behaviors such as wandering or inappropriate/disruptive behaviors
  • Require ongoing care and supervision
  • Have a signed order for AFC services from their primary care physician
  • Have a recent physical examination (within the past 3 months)
  • Have two qualified caregivers

Qualified Caregiver Requirements

Caregivers may be family members or non-family members. However, a caregiver cannot be:

  • A spouse
  • A parent of a minor child
  • A legal guardian
  • Any other legally responsible relative

To qualify, a caregiver must:

  • Be at least 18 years of age
  • Be willing to live with the client and provide necessary care
  • Be a relative, friend, or trusted member of the community
  • Agree to provide assistance, supervision, and hands-on care according to the client’s individualized care plan
  • Ensure the client’s safety and overall well-being at all times
  • Agree to and pass a CORI (criminal background) check

Qualified Setting

  • The care setting must be located in the state of Massachusetts
