Adult Foster Care (AFC) Program Requirements
Our state-funded Adult Foster Care program enables seniors and individuals with disabilities to remain in the comfort of their homes, supported by a dedicated caregiver who provides personalized care.
Eligible Participants
To participate in the Adult Foster Care program, individuals must:
- Have MassHealth or CommonHealth insurance
- Be at least 16 years of age
- Have a medical diagnosis or require assistance with one or more Activities of Daily Living (ADLs), including:
- Bathing
- Dressing
- Eating
- Toileting
- Transferring (in/out of bed; up/down from a chair)
- Ambulation
- Management of behaviors such as wandering or inappropriate/disruptive behaviors
- Require ongoing care and supervision
- Have a signed order for AFC services from their primary care physician
- Have a recent physical examination (within the past 3 months)
- Have two qualified caregivers
Qualified Caregiver Requirements
Caregivers may be family members or non-family members. However, a caregiver cannot be:
- A spouse
- A parent of a minor child
- A legal guardian
- Any other legally responsible relative
To qualify, a caregiver must:
- Be at least 18 years of age
- Be willing to live with the client and provide necessary care
- Be a relative, friend, or trusted member of the community
- Agree to provide assistance, supervision, and hands-on care according to the client’s individualized care plan
- Ensure the client’s safety and overall well-being at all times
- Agree to and pass a CORI (criminal background) check
Qualified Setting
- The care setting must be located in the state of Massachusetts
